Requirements

For Corporate or Partnership

  • Certificate of Registration (DTI, SEC, BSP)
  • Secondary license or certificate of authority
  • Articles of Incorporation/Partnership
  • Latest GIS which lists the names of directors/trustees/partners, principal stockholders owning at least twenty percent (20%) of the outstanding capital stock, and primary officers such as the President and Treasurer; if applicable
  • Board or Partner?s resolution duly certified by the Corporate/Partner?s Secretary authorizing the signatory to sign on behalf of the entity
  • Pictures and IDs for all authorized signatories

For Associations and Non-profit Organizations

  • Board Resolution duly certified by the Corporate/Partner?s Secretary authorizing the signatory to sign on behalf of the organization
  • By Laws/Articles of Incorporation/Association (if applicable)
  • Pictures and IDs for all authorized signatories
